DEVELOP SYSTEMS FOR HOUSEKEEPING STANDARDS

Implement sustainable systems to establish and maintain good house standards. Over 54% of injuries in the SA Metalliferous Mining industry in the last decade are attributed to general injuries, where the major risk agents driving these injuries are slip/trip & fall as well as hand injuries.

Hand injuries are often due to tools not fit for purpose, while slip and fall accidents / incidents are solely caused by poor housekeeping conditions, which are completely preventable.

DEVELOP AND IMPLEMENT PTO MANAGEMENT MATRIX

This is the most effective tool that improves employee competence for all those skills acquired through on the job training, the tool further gives an historical record on the account of employee competency and experience.

The matrix must cover all employees and allow for job observation of each high risk tasks an employee performs at a frequency aligned to the likely hood of safety incidents associated with the task. At most one task or procedure must be evaluated from each employee in 1 calendar year.

DEVELOP A PRACTICAL BASELINE RISK ASSESSMENT

Developt a practical (and not a conceptual) Baseline Risk Assessment which can be integrated into the risk control matrix of the
operation which includes

  • PEOPLE - Safety training programs
  • EQUIPMENT - Operating procedures
  • SYSTEM - Codes of practice and safe work procedures

IMPLEMENT EFFECTIVE TRAINING MATRIX SYSTEM

Implementation of effective training matrix system that eliminates employee non-available shifts due to expiry of safety training. i.e.

  • Working at heights
  • Isolation and lockout
  • Confine space
  • First Aid training etc
  • Basic rigging and safelifting
  • Overhead crane operation
